Our client is a modern and innovative Spitex organisation with around 125 dedicated employees. The focus is on professional, individual, and holistic care and nursing of people in their familiar home. In doing so, Spitex places great importance on high quality of care, respectful interaction, and appreciative cooperation within the team and with various healthcare professionals.

Your tasks

• You ensure professional, individual, and holistic care and support of our clients in their own homes.
• You care for people with different clinical pictures and contribute your expertise, particularly in the areas of somatics, wound care, palliative care, and dementia.
• You independently take over case management and coordinate the entire nursing process responsibly and purposefully.
• You carry out needs assessments, plan the required nursing measures, and document the nursing process carefully in InterRAI Home Care.
• You recognize changes in health status early and initiate corresponding measures.
• You work closely and professionally with nursing experts, doctors, hospitals, as well as other professionals and institutions.

Your profile

• Completed training as dipl. Pflegefachperson HF or FH
• Independent, competent, and reliable personality with a strong sense of responsibility
• Good user knowledge in the field of IT
• Driver's license category B
• Experience in the Spitex sector
