Care with Responsibility – Dipl. Pflegefachperson HF/FH (m/f/d) sought in Zürich

Do you want to use your nursing expertise specifically in assessments, counseling, and care planning? For our client in Zürich-Oerlikon, we are looking for a diplomierte Pflegefachperson HF/FH who values independent work and provides professional guidance to people and their relatives.

Your focus is on the assessment of care needs, the planning of suitable measures, and the coordination of the care process. Depending on experience and the agreed scope of tasks, you will also take over case management.

Your tasks

• You carry out nursing assessments and assessments.
• You create individual care plans, adapt them, and check their effectiveness.
• You assess nursing situations and document your assessments in a comprehensible manner.
• You advise persons under care and support nursing relatives in everyday life.
• You coordinate, if necessary, the cooperation with medical and other specialists.
• You handle administrative tasks related to the care process.
• You contribute your specialist knowledge in complex nursing situations.

Your profile and your strengths

• Completed training as dipl. Pflegefachperson HF/FH or an equivalent recognized qualification
• Experience in nursing assessments, care planning, or case management is an advantage
• Independent, structured, and responsible way of working
• Empathy and enjoyment in advising people
• Good German skills in speaking and writing
• Albanian language skills particularly desired to advise the persons under care and their relatives
• Confidence in handling digital documentation and administrative

tasks

• Driver's license category B
