We are looking for the CMS of Payerne at a rate between 60%-80%, a: ASSISTANT IN CARE AND COMMUNITY HEALTH (ASSC) at 60%-80% Mission: * Collaborate on the medico-social care project (support in the form of assistance and care) based on respect for the principle of the person's autonomy. * Participate in monitoring the evolution of the client's situation, in collaboration with the client, the entourage, the interdisciplinary team and the treating physicians. * Provide clients with services such as basic care, nursing care, household help and family assistance. Profile: * Hold a CFC as ASSC. * Have, if possible, experience in home care. * Be available to work during the day, in the evening and on weekends. * Be able to work autonomously and in an interdisciplinary manner. * Hold a driver's license and possess a vehicle (possibility of using a professional vehicle). * Knowledge of German would be an asset.
