We are looking for

Social worker for home-based socio-educational follow-ups (60 to 80%)

Immediately or by agreement

We offer you

• ensuring the support of clients at their homes in daily gestures and activities according to the institutional rehabilitation concept, employee regulations, and safety standards;
• developing documented individual projects with their referrals while guaranteeing the interprofessionalism of the projects. Guaranteeing their follow-up;
• managing the individualized follow-up file of their referrals;
• collaborating with the network of their referrals.

Your profile

• you hold a HES / ES diploma or training deemed equivalent. A CAS in psychiatry is an asset;
• you have confirmed work experience in the field of psychiatry or social work in a residential home and/or in home-based follow-up;
• endowed with a very good sense of organization, you like to work autonomously and in a team and to be able to assume responsibilities;
• you master the usual MS Office IT tools;
• holder of a driver's license, you have a private vehicle.
