What you bring with you

• You play a decisive role in ensuring that our customers receive professional and individual care at home
• You conduct needs assessments and create an individual care plan
• You coordinate the necessary nursing measures and ensure cooperation with relatives, doctors, and other involved specialist agencies.
• You perform nursing and medical-technical measures professionally and independently
• As a case-leading employee, you manage the care process of your assigned customers, initiate situation-appropriate interventions, ensure professional and complete documentation, and actively participate in interprofessional cooperation.
• You are in close exchange with the nursing service management and actively participate in measures for quality assurance and quality development

What you bring with you

• You have a diploma as a Pflegefachfrau / Pflegefachmann HF or an equivalent, recognized training in Switzerland
• You have an InterRAI-HC course or are willing to complete it
• You enjoy working independently, are flexible, and organize your daily work reliably
• You have good oral and written German skills and can communicate confidently with customers, relatives, and specialists
• You are characterized by empathy, sense of responsibility, and resilience
