Your tasks

• Comprehensive care and global care of hosts in the spirit of our care model and the care process
• Taking on tasks as a care reference person
• Responsibility and control of the care process of assigned hosts, including the classification of care needs RAI/BESA/PLAISIR
• Collaborating with external stakeholders (doctor, physio…)
• Planning and accompanying medical visits
• Participation in care reports and transmission of important observations
• Support for personal care, movement, and other daily activities Organization and proposal of daily activities for our hosts with creative occupations, walks, etc.
• Writing care notes in our care documentation system CareCoach (possible via mobility with the CareCoach smartphone)
• Taking on daily responsibility with coordination of the daily team
• Management of case discussions and nursing care reports
• Taking on tasks as a theme bearer in a specialized theme and thus participating in the quality network of care at the group level

Your profile

• Completed HES certified nurse training or equivalent training recognized by the CRS / Completed CFC training as an assistant in community nursing and health care
• Several years of professional experience in care is an asset
• RAI/BESA/PLAISIR knowledge desired or willingness to follow these courses (paid)
• Organized, calm, rigorous, committed and motivated
• A warm attitude full of humor
• Pleasure in taking on varied challenges
• French native speaker or practicing it fluently
