What you can achieve ###

• Responsible for the planning and coordination of discharges and transfers in close cooperation with interdisciplinary treatment teams
• Control and optimize processes in discharge management and continuously develop case management
• Organize and coordinate individual follow-up solutions in the outpatient and inpatient sectors
• Advise and accompany patients, relatives as well as treatment teams in complex care situations
• Ensure holistic accompaniment of patients throughout the entire course of treatment

What you bring with you ###

• Completed training as a nursing professional HF/FH as well as ideally further training in case management or the willingness to complete it
• Several years of professional experience in acute medicine and sound understanding of clinical processes
• Confident handling of MS Office and digital work tools
• Strong communication skills, high service orientation and social competence
• Organizational skill and assertiveness as well as enjoyment of interprofessional cooperation and change processes
