We are looking for the CMS of Avenches a: Assistant in community nursing and care at 60% - 80% Mission: * Collaborate on the medico-social care project (support in the form of help and care) based on respect for the principle of the person's autonomy. * Participate in monitoring the evolution of the client's situation, in collaboration with the client, the entourage, the interdisciplinary team and the treating physicians. * Provide clients with services such as basic care, nursing care, household help and family help. Profile: * Hold a CFC as ASSC. * Have, if possible, experience in home care. * Be available to work during the day, in the evening and on weekends. * Be able to work autonomously and interdisciplinarity. * Hold a driver's license and possess a vehicle (possibility to use a professional vehicle). * Knowledge of German would be an asset.
