To complement our team, we are looking for the Discharge and Social Counseling department as of 1 October 2026 or by arrangement for a:

Social Worker 50-80 % ###

Your challenge ##

• Counseling of patients including the involvement of relatives during the hospital stay to prepare for discharge, mediation of follow-up solutions
• Interdisciplinary cooperation in the care of seriously ill and dying patients and their relatives
• Mediation of external specialist services and medical aids
• Counseling of patients in insurance and legal matters including clarification of financing questions, accessing financial support or untapped financing sources
• Obtaining cost guarantees in the country and abroad
• Interdisciplinary cooperation with all nursing stations and the medical and nursing service
• Cooperation and networking with aftercare institutions
• Participation in hospital projects

Your profile ##

• Completed degree in Social Work FH or equivalent
• Experience in the healthcare sector, social service, case management, or basic training in nursing is an advantage
• Very good communication skills
• Solution-oriented handling of crises and problem situations
• Strong social competence and empathy
• Willingness to work interdisciplinary
• Eager to learn and interested in completing training/further education
• Professional handling of public services
• Motivated and committed personality characterized by an independent way of working and able to make decisions independently
