The CMS READOM is an innovative program specialized in home rehabilitation. The multidisciplinary team intervenes for a maximum of three weeks. It is composed of occupational therapists, nurses, ASSC, physiotherapists, and health assistants. It collaborates with the client, their surroundings, and the care network. In order to complete our team, we are looking for an Assistant in community nursing and health 80% LOCATION CMS de READOM YOUR MISSION * Perform medico-delegated care at the home of people presenting a state of physical or mental dependency or a disability. * Support the CMS multidisciplinary team in the management of client situations. * Integrate health promotion and prevention concepts into practice.
