Your tasks
Professional and individual care and support of the residents on the conceptual basis of the model of promoting process care
Planning, implementation and evaluation of nursing measures in compliance with the criteria of the nursing process and involving the affected residents and their contact persons.
Co-shaping of optimal interdisciplinary cooperation with all services involved in the nursing and care process
Assumption of daily responsibility
Careful and complete maintenance of the nursing documentation
Exact recording of nursing and care services as well as the nursing and consumable materials used. RAI responsibility:
Execution and ensuring of timely RAI assessments according to legal and operational specifications.
Responsibility for the correct classification of the nursing needs as well as the nursing classification of the residents.
Creation, review and continuous updating of the individual nursing plan based on the assessment results.
Ensuring complete, professional and traceable nursing documentation.
Advice and support of the nursing specialists in the application of the RAI system and nursing planning.
Monitoring of assessment dates as well as ensuring data quality and completeness of the documentation.
What you bring with you
Completed vocational training as a certified nursing specialist (HF, FH, AKP, DNII, DNI, PSYKP) or equivalent diploma recognized by the SRK
Experience in long-term care and in dealing with older people
Experience with the RAI - system
User knowledge of the software CareCoach (nursing documentation) is an advantage
Empathic and resilient personality with high commitment
Very good written and oral German skills
