• Your field of activity
• You are responsible for optimal care and advice for our clients with a focus on dementia
• With case management, you take responsibility for all steps in the nursing process of your clients
• You undertake coordination tasks with other involved services and actively participate in the cooperation with our interprofessional team.

• Completed training as a certified nursing specialist (DNII / HF / FH)
• Experience in the nursing and care of people with dementia
• Empathic, innovative, flexible and cheerful personality with strong social, communication and self-competencies
• Flexibility and resilience
• Driver's license Cat. B
