Das Gesundheitszentrum Dielsdorf is a modern geriatric competence center with around 500 employees from 35 nations. Our range of services extends from advisory services for elderly people and their relatives to general and specialized long-term care and to care and support at home. As the largest public Spitex in the Dielsdorf district, we provide outpatient services for our nine contract municipalities. With our comprehensive nursing offer, we enable people to stay in their familiar home for as long as possible despite health restrictions. In the future, it is planned to establish a night Spitex.

Your tasks

• Nursing, care and advice for our clients at home
• Organization and structuring of your work during the day from Monday to Friday
• Responsibility for conducting assessments with Inter RAI-CMH, psychosocial nursing and care of chronically and mentally ill people as well as conducting conversations and recovery-oriented support of therapeutic processes
• Intensive exchange, also beyond the boundaries of the Gesundheitszentrum Dielsdorf, with relatives, social workers, the KESB, specialist agencies, doctors etc.
• Active participation in the setup work and further development of the psychiatry Spitex

Your profile

• Completed training as Dipl. Pflegefachperson DN II/HF/FH
• Several years of professional experience in the field of psychiatry
• High social, methodological and specialist competence
• Very good German language skills in speaking and writing
• Modern IT skills (MS Office, Perigon)
• Driver's license Cat. B, own car desired
• Independent and responsible way of working
