Pflegefachperson HF/FH (50-100%) immediately or by arrangement. Your task You take on professional responsibility and accompany our customers holistically in their familiar living environment. Assessment of needs and case management are essential components of your area of responsibility. Your tasks include in particular: Carrying out assessments of needs as well as taking over case management Holistic nursing and care of our customers at home Advising customers as well as their relatives regarding home care Close cooperation with family doctors as well as other partner organizations Professional guidance and support of employees in the interdisciplinary team Active contribution to high-quality and needs-oriented care You enjoy working independently and taking responsibility - at the same time, you can count on professional exchange and the support of a committed team.
