Siloah Langzeit offers comprehensive care to elderly people in need of care, people with disabilities, and persons with special needs at the Gümligen, Kühlewil, and Köniz locations. The offering includes geriatric care in elderly residential groups – including specialized offers for people with dementia, addiction diseases, or gerontopsychiatric needs – as well as care in neurological residential groups. ### Your area of responsibility

• Coordination and support of the recording of the individual help plan (IHP)
• Responsible for the coordination and professional leadership in the implementation of the new services for people with disabilities
• Support for employees on professional questions regarding the IHP process
• Conducts needs assessments in cooperation with the social pedagogues in the residential groups
• Participation in the further development of processes and standards in the area of IHP
• Cooperation and coordination with external agencies, especially with cantonal authorities and other involved specialist agencies
• Consulting and professional support for residents, legal guardians in connection with individual help planning

Your profile:

• Completed training in Social Pedagogy HF/FH or Social Work FH; alternatively, a psychology degree with relevant practical experience in the social sector
• Good, application-oriented specialist knowledge and sound experience in working with people with mental impairments, IV and AHV benefits, as well as special support needs
• Knowledge in interdisciplinary cooperation with nursing, legal guardians, relatives, and therapeutic specialist agencies as well as in dealing with relevant legal foundations
• Enjoyment of conceptual work
• Empathic and respectful attitude, strong team, communication, and reflection skills as well as an independent, resilient, and flexible way of working
