Responsibilities

• Ensuring efficient, demand-oriented, and sustainable discharge and transfer planning with the goal of guaranteeing the quality of care and avoiding unnecessary readmissions and rehospitalizations
• Holistic case management of patients during the inpatient stay, taking into account the individual social, medical, and care-related situation
• Conducting and coordinating interdisciplinary discussions with patients, relatives, as well as internal and external specialists
• Planning, organization, and coordination of transfers to rehabilitation, spas, nursing institutions, or home care
• Networking and collaboration with internal and external service providers, authorities, insurance companies, and other involved agencies

Requirements

• Training in the health, social, or a related field, ideally with further training in case management
• Professional experience in the healthcare sector as well as experience in interprofessional collaboration
• Experience in advising and supporting people in stressful life situations
• Strong communication, coordination, and organizational skills
• Independent, solution-oriented, and networked way of working
• Knowledge in the area of social insurance
• Enjoyment of working with patients, relatives, as well as internal and external specialists
