In close collaboration with the multidisciplinary team and the situation referents, you will play a central role in the implementation and evaluation of care objectives. Your main missions: * Perform delegated medical care, basic care, and household help in compliance with prescriptions and individualized needs. * Observe, evaluate, and transmit changes in the health status of beneficiaries in order to adapt the care provided. * Support relatives and families, and contribute to interdisciplinary coordination. * Actively participate in the continuous improvement of the quality of care and the sharing of practices within the team. * Contribute to maintaining staying at home and to the promotion of community health.
