This opportunity could interest you. ### Your mission In the context of your functions, you intervene directly at the patients' homes and contribute to offering them support adapted to their needs. Your main responsibilities will notably include: * Ensure care and support for patients at home while respecting their habits and autonomy; * Perform basic care and medico-technical acts within your field of competence, upon delegation; * Accompany patients in daily life activities; * Observe the general state of the patient and be attentive to any evolution of their situation; * Transmit relevant observations to the nursing team; * Participate in the continuity and quality of care; * Promote the maintenance of autonomy and well-being of the people supported; * Collaborate with nurses, doctors, and other stakeholders in the healthcare network; * Ensure rigorous documentation of care and interventions; * Respect hygiene, safety, and confidentiality rules and the procedures in force. Working at home also requires a certain autonomy in the organization of interventions, while maintaining good communication with the team. ### Your profile We are looking for a person: * Holder of a CFC as Assistant in community nursing and health (ASSC) or a title recognized as equivalent; * Comfortable in contact with patients and their relatives; * Benevolent, respectful, and attentive to the needs of each person; * Autonomous and capable of taking initiatives within the framework of their competence; * Organized and capable of managing priorities; * Possessing a good sense of observation and good communication skills; * Appreciating teamwork and interdisciplinary collaboration; * Flexible regarding schedules and the different types of interventions; * Holder of a driver's license and possessing a vehicle, indispensable for travel between homes.
