In close collaboration with the multidisciplinary team and the situation referents, you will play a central role in the implementation and evaluation of care objectives. Your main missions: * Perform delegated medical care, basic care, and household assistance in compliance with prescriptions and individualized needs. * Observe, evaluate, and transmit changes in the health status of beneficiaries in order to adapt care. * Support relatives and families, and contribute to interdisciplinary coordination. * Actively participate in the continuous improvement of the quality of care and the sharing of practices within the team. * Contribute to staying at home and to the promotion of community health Location: Aigle site Profile: * CFC as ASSC - Assistant in care and community health * Sense of responsibility and professional ethics * Great sense of relationship, communication, and negotiation * Autonomy, versatility, flexibility, and adaptability * Proactivity, reactivity, and solution orientation * Good capacity for analysis and evaluation, as well as collaborating in networks and interdisciplinarity * You are autonomous, dynamic, and enjoy working in a team * Comfortable with IT tools (management of patient files, care planning, monitoring of quality indicators). * Driver's license and vehicle indispensable * For foreign diplomas, a Croix-Rouge recognition is required or in progress.
