Your tasks

• Holistic care and nursing of guests in the sense of our nursing mission statement and the nursing process
• Taking on tasks as a primary contact person for nursing
• Responsibility and management of the nursing process of the assigned guests including the nursing needs assessment RAI/BESA
• Electronic nursing documentation using CareCoach
• Planning and accompaniment of physician visits
• Taking on daily responsibility with coordination of the day team
• Leading case discussions and nursing reports
• Taking on tasks as a subject matter expert in a specialist topic and thus participating in the group-wide network for nursing quality
• Support and advice for employees regarding questions about RAI LTCF and the nursing process
• Recognizing changes in nursing and care needs as well as timely adjustment of the corresponding planning
• Participation in case discussions, nursing visits, and interprofessional meetings
• Promotion of resource-oriented and person-centered nursing
• Active participation in securing and further developing our nursing quality

Your profile

• Completed training as a Dipl. Pflegefachfrau/Pflegefachmann HF/FH/DNII or an equivalent training recognized by the SRK
• Completed training as a Fachfrau/Fachmann Gesundheit EFZ
• Professional experience in long-term care is an advantage
• Profound practical experience with RAI LTCF
• Professional experience in Spitex/Inhouse Spitex is desirable
• Very good knowledge of the nursing process and professional nursing documentation
• Confidence in the implementation and assessment of assessments
• Strong sense of quality and responsibility
• Independent, structured, and reliable way of working
• Joy in passing on specialist knowledge and supporting colleagues in everyday work
• Appreciative and respectful attitude towards our guests and employees
• Good German skills in speaking and writing as well as confidence in dealing with digital documentation systems
