The Fondation Pré Pariset brings together three medico-social establishments (EMS) located on 3 sites in the city of Pully, with a total of 132 beds. It employs 240 collaborators. Our mission is to accompany elderly people experiencing a loss of autonomy, in long and short stays, as well as in its temporary reception center (CAT). We also manage a public restaurant, "le Delta", with 54 places and a terrace.

To complete the CAT animation team and collaborate actively with the long-stay animation team, we are recruiting for the position of

Social Animator - Field Coordinator at 60%

Main Missions

• As a member of the temporary reception center (CAT) team, actively participate in its daily functioning, being present in the field with the beneficiaries.
• Design, organize, and directly animate CAT activities, promoting participation, social interactions, maintenance of abilities, and the well-being of beneficiaries.
• Organize and carry out outings for beneficiaries
• Welcome, listen to, and accompany beneficiaries in their journey, respecting their dignity and individuality.
• Evaluate the needs of beneficiaries and direct them toward appropriate social resources (housing, health, etc.), in a perspective of maintaining life at home or, when the time comes, transition.
• Implement individualized support projects in collaboration with members of the interdisciplinary team and the beneficiaries.
• Collaborate with local partners (social services, associations, institutions, CMS, etc.) to ensure global care for beneficiaries and their maintenance at home.
• Support family caregivers through counseling and orientation actions
• Ensure the preparation and follow-up of beneficiary files.
• Participate in the coordination of actions with multidisciplinary teams and contribute to the improvement of professional practices.
• Guarantee respectful and benevolent accompaniment, in accordance with the values of the Foundation
• Coordinate and carry out the CAT team's work schedules, ensuring the proper organization of activities and continuity of support.
• Actively promote the CAT to partners (doctors, pharmacists, etc.)
• Welcome beneficiaries, accompany visits and integration days
• Be a source of proposals for the development of the CAT and its activities.

Profile

• HES/ES degree in social work or recognized equivalent title (sociocultural animation option preferred), mandatory
• Confirmed experience in a similar function
• Willingness and interest to invest oneself within the CAT team on a daily basis
• Ability to carry out complete support (including ADLs)
• Excellent knowledge of the social network and assistance systems (social insurance, social legislation, etc.).
• Great capacity for listening and communication
• Ability to work autonomously while integrating into a multidisciplinary team.
• Sense of organization, rigor, and adaptability.
• Benevolent and empathetic personality
• Excellent mastery of IT tools (Microsoft office, SADIES suite, etc.)
• Driver's license mandatory
