Our Reduced Mobility transport office (TMRL) of the Lausanne region has the mission to coordinate transport for people with mobility difficulties. This office brings together the home care organizations of the Lausanne agglomeration (Fondation Soins Lausanne, APROMAD, APREMADOL) and Pro Infirmis.

In order to complete our teams following the departure of a collaborator, the CMS of Fondation Soins Lausanne is looking for a:

Occupational Therapist (M/F)

Activity rate 20%

Permanent indefinite contract

Your mission

• Record and ensure the follow-up of evaluations carried out by the CMS.
• Speak with the evaluators (occupational therapists, nurses and social workers) of the CMS of Lausanne and surroundings and of Pro Infirmis when evaluations are incomplete or present inconsistencies.
• Validate the assignment of one or more transport providers for clients with reduced mobility.
• Ensure each month the update and transmission of re-evaluation lists for the three coming months to the CMS and to TMRL respondents.
• Offer an accompaniment and training service to evaluators through regular presentation of TMRL services.
• Offer a training service to drivers intervening within the framework of TMRL trips.
• Collaborate, in an advisory capacity, with the TMRL manager in improving the functioning of the TMRL Office regarding evaluations.

Your profile and your skills

• Bachelor in occupational therapy or title deemed equivalent (foreign diploma recognized by the CRS)
• Authorization to practice or ready to start the procedures - conditional on engagement
• Experience in hospital service for physical care or psychiatry, rehabilitation service, home care or private practice an asset
• Marked interest in community health
• Ability to collaborate in networks and multidisciplinarity
• Sense of responsibility, ability to organize oneself autonomously and to set priorities
