For the Spitex area, we are looking for a responsible personality as Dipl. Psychiatriefachperson HF with case management, starting by agreement.

Tasks

• Diploma as Pflegefachperson HF/FH, at least 2 years of professional experience in a psychiatric setting
• Joy and interest in the psychosocial care of older people
• Independent and responsible way of working
• Knowledge of InterRai CMH and Perigon is an advantage
• Driving license Cat. B

Profile

• As a case-managing nursing specialist, you manage and control all steps in the nursing process.
• You are responsible for careful and complete nursing documentation.
• You provide resource- and goal-oriented care for our clients with mental illnesses.
• You support our clients in designing and implementing a self-determined everyday life.
