YOUR TASKS

• You advise and inform on social, personal, financial, and legal questions in the context of the hospital stay and discharge planning. Additionally, you organize tailor-made follow-up solutions, provide practical assistance if necessary, and triage to appropriate specialist centers
• You maintain contacts and cooperations with external institutions and service providers
• You can expect close interdisciplinary cooperation with the entire medical, nursing, and therapeutic treatment team
• You have an active role in the co-design and further development of the Case Management service for our patients

YOUR PROFILE

• Completed training as a Dipl. Pflegefachperson with a wealth of experience
• Ideally, you have experience in case management in an acute hospital or even have completed further training in case management
• Good knowledge of the health and social insurance system
• Empathic and integrating personality with high social competence
• You enjoy working independently, but also in a team. You are solution-oriented and take responsibility
• You enrich our team with your humorous and cheerful manner
