Nursing Professional HF/FH

Our client is an established non-profit organization in the field of nursing, advisory, and care services. It supports people of various age groups in leading a self-determined life in their familiar environment despite health restrictions. With around 160 employees at several locations, the organization is one of the larger providers in the field of outpatient care and impresses with a dynamic working environment as well as a motivated and collegial team. To strengthen the team, we are looking for an engaged and professionally competent personality as a Nursing Professional HF/FH, starting immediately or by arrangement.

Your tasks

• Carrying out needs assessments according to interRAI-HC
• Independent nursing and care of clients
• Taking over case management
• Assessment of care needs and the care effort
• Creation and evaluation of nursing documentation
• Execution and documentation of interventions according to nursing planning
• Conducting advisory consultations
• Verification of the quality and effectiveness of the nursing care
• Cooperation with relatives, doctors, and specialist agencies

Your profile

• Completed training as a Nursing Professional HF/FH
• Extensive professional experience in nursing
• Professional and independent way of working
• Strong social competence and resilience
• Flexibility regarding working hours
• Good PC skills
• Driving license category B
