• You ensure professional, individual, and holistic care and support of our clients in their homes.
• You take care of people presenting different pathologies and bring your specialized knowledge, notably in the fields of somatics, wound care, palliative care, and dementia.
• You autonomously assume case management and coordinate the entire care process in a responsible and targeted manner.
• You perform needs assessments, plan the necessary care measures, and carefully document the care process in InterRAI Home Care.
• You detect changes in health status early and take appropriate measures.
• You work in close and professional collaboration with care experts, doctors, hospitals, as well as other professionals and institutions.
• Completed training as a registered nurse HF or FH
• Autonomous, competent, and reliable personality with a keen sense of responsibility
• Good IT knowledge
• Driving license category B
• Experience in the Spitex field
