Our client is looking for a Dipl. Pflegefachperson HF/ FH for home assignments starting from 01. August 2026 for at least 3 months - with the option to extend. The non-profit organization supports people in somatic and psychiatric care as well as Palliative Care. In this temporary role, you will undertake important nursing services, accompany the daily lives of the persons under care, and ensure safety.

Your tasks with the new employer:

• You care for and support our clients directly in their homes and ensure individual and dignified care
• You take responsibility for the professional planning and implementation of the entire nursing process with case management
• You determine nursing and care needs using the interRAI HC instrument, perform reassessments, and adapt the nursing plan according to needs
• You are the contact person for relatives, general practitioners, pharmacies, as well as social service providers and participate in cooperation and coordination

You offer your new employer:

• You have completed training as a dipl. Pflegefachperson HF / FH
• You are a reflective, open-minded, and flexible personality with empathy, patience, and the ability to maintain an overview even in difficult situations
• You work independently, in a structured manner, and with high personal responsibility - ideal for assignments with high personal responsibility
• You possess good German skills in speaking and writing as well as a driver's license category B
