We are looking for our various sites (Delémont, Saignelégier, Courroux, Bassecourt, and Porrentruy) and for an indefinite period, : Community health and nursing assistants (50%-80%) Your mission: * You take charge of patients requiring home care and perform medico-technical acts and individualized and relational basic care upon delegation from nurses. * In collaboration with nurses, you participate in care objectives aimed at maintaining the maintenance, mobility, and autonomy of patients at home. * You evaluate home care situations and adapt services to the environment of the patient and their surroundings, in simple and complex situations. * You provide patients with support in activities of daily living, respecting criteria of safety, efficiency, comfort, and hygiene. Your profile: * You hold a diploma as a community health and nursing assistant (ASSC) or an equivalent title. * You demonstrate autonomy, a sense of observation, and great respect for professional and functional secrecy. * You enjoy teamwork and collaborate in a way that transmits your observations both orally and in writing.
