Medical Practice Assistant with focus on Chronic Care Management (m/f/d)

For our client, an established company in the field of outpatient medical care in the St. Gallen region, we are looking for an engaged medical professional, per immediately or by arrangement, who is at home in both classic MPA activities and brings joy to the care of chronically ill patients.

Tasks:
In this versatile part-time position, you will combine two fields of activity in a meaningful way. One to two days per week, you will conduct your own consultation hours in the field of Chronic Care Management and competently and empathetically accompany people with diabetes as well as other chronic illnesses. Close cooperation with general practitioners is as much a part of your everyday life as active participation in training sessions, quality circles, and the continuous further development of existing treatment programs. On the remaining working days, you will work in the regular MPA rotation and take on responsibility in various medical specialist areas. Structured processes and modern software solutions will support you in your daily work. Emergency services are waived, and any overtime incurred can be fully compensated.

Requirements:
You have completed training as a Medical Practice Assistant and ideally already bring experience or particular interest in the field of Chronic Care Management. Strong communication skills as well as the willingness to also accompany patients in an advisory capacity distinguish you. You work independently, structured, and reliably, but at the same time value collegial exchange in a supportive team. The confident use of digital work tools is a matter of course for you, and you bring the motivation to continuously develop professionally.
