Your tasks:

• You will take over assignments in basic and treatment care with our customers.
• You play a decisive role in ensuring that our customers enjoy first-class and individual care at home.
• You advise and accompany our customers and their relatives based on their individual needs.
• You conduct needs assessments with RAI-HC at the customer's location.
• You participate in preventive, diagnostic, and therapeutic measures.
• Promotion of the independence and quality of life of customers in the home environment.
• You perform comprehensive nursing histories and assessments.
• You instruct subordinate employees regarding nursing measures.
• You take over administrative tasks within the scope of case management.

What you bring with you:

• You are a certified nursing professional (HF, FH, AKP, DN II).
• You have the InterRai HC course or are willing to complete it.
• Ideally, you have several years of professional experience in long-term care or the Spitex sector.
• Independent, structured, and efficient working distinguishes you.
• You are resilient and flexible.
• You have a category B driver's license and ideally have your own car.
