Our client, a non-profit Spitex organization in Limmattal, provides comprehensive outpatient care on behalf of the municipalities. A dedicated specialist team is in operation daily and around the clock - with the goal of ensuring the quality of life of the clients and strengthening their independence in their familiar environment.

Job Description ##

• You are active in direct care and, in complex care situations, take on the consultation and guidance of customers, relatives, and employees
• You carry out needs assessments using the InterRAI instrument
• As a case manager, you are responsible for the planning and implementation of the nursing process for your customers
• You play an active role in cooperation with the supporting, interdisciplinary network and coordinate the corresponding services for your customers

Requirement Profile ##

• You have a degree as Dipl. Pflegefachfrau HF / FH as well as ideally corresponding professional experience in this field
• You are an open-minded and flexible personality who enjoys approaching people
• You bring good communication skills as well as confident handling of mobile nursing documentation (smartphone)
• You think in networks, work independently and responsibly
