It brings together healthcare providers (hospitals, EMS, CMS, doctors, etc.) and municipalities at the regional level. Its mission is to improve, in close collaboration with its members, the coordination of care and the orientation of users in the healthcare system. The RSLC manages the BRIO, whose main missions are: the orientation of people losing autonomy upon hospital discharge, the management and evaluation of requests for accommodation in EMS and information on medico-social services to the population and professionals. To complete its team, the Réseau Santé la Côte is looking for a coordination nurse Activity rate: 50 % - Indefinite term contract Start date: 1er janvier 2027 or to be agreed Within the BRIO, your activity: - Respond to requests for accommodation in EMS and in other structures of the system depending on availability. - Evaluate requests and propose to the applicant solutions in accordance with their needs and wishes. - Participate in and organize network meetings when situations require it. - Centralize and disseminate information on the services of the health and medico-social institutions of the region and the canton. - Provide information, support, advice and guidance to the population and professionals of the network. - Participate in and represent, by delegation, the Réseau Santé La Côte in working groups (BRIO activity development project for example). Your profile: - Bachelor in nursing at HES level or equivalent. - Certifying postgraduate training in support and care for the elderly is welcome. - Professional experience in the care of elderly people. - Good knowledge of the Vaud medico-social system and its institutions. - Ability to negotiate, listening skills and empathy, particularly for complex situations. - Pleasure in working in a team. - Enjoying being in contact with an important network of regional and cantonal partners. - Experience or training in psychiatry is welcome. - Mastery of the French language (written and oral). - Vehicle and driver's license required.
