Mission

• Ensure care and appropriate management for people of all ages, in diverse contexts, in collaboration with their entourage.
• Participate actively and in a multidisciplinary manner in the organization and follow-up of the care project defined with the client.

Profile

• Bachelor HES-SO in Nursing or equivalent or foreign diploma recognized by the Swiss Red Cross
• Skills in managing a patient portfolio based on continuous evaluation and monitoring of care objectives
• Knowledge of the home care field, an asset
• Ability to collaborate within a multidisciplinary team
• Listening skills and helping relationship skills
• Ability to be proactive and to position oneself as a partner in network work
• Ability to adapt to change, to function autonomously and to manage complex and/or crisis situations
• Available, flexible and good adaptability
• Driving license indispensable
