As a member of the PCLD, you will be required to intervene throughout the Canton of Geneva for long-term replacements, varying from 6 to 15 months, within our branches. Your main mission will consist of ensuring the continuity of patient follow-up, acting as a reference in the management of their files and providing quality services.

By joining the PCLD team, you will be able to:

• Assume the role of reference in assessing patient needs, in accordance with the ethical principles of the nursing profession, within the home care team model.
• Provide personalized nursing care, autonomously or upon medical delegation, covering a full range of preventive, curative, educational, and palliative services, directly at the homes of patients of all ages.
• Work in close collaboration with community care and health professionals as well as multidisciplinary aides, delegating care according to established procedures.
• Organize medico-social steps and ensure coordination with all stakeholders involved in a given patient's situation, by mobilizing the available resources of the primary and secondary networks.

Qualifications

• A Bachelor HES-SO in nursing or a training deemed equivalent, as well as recognition from the Croix-Rouge.
• An excellent ability to adapt and delegate, with demonstrated flexibility and availability.
• A sense of responsibility and a solution-oriented approach, essential for meeting the challenges encountered.
• An aptitude for working autonomously and interprofessionally within a multidisciplinary team.
• Mobility is indispensable, which may involve traveling on foot, by bicycle, by public transport (TPG) or by car depending on the missions.
