We are looking for the CMS de Moudon, a: Assistant in community nursing and healthcare at 50% - 80% Mission: * Collaborate on the medico-social care project (support in the form of help and care) based on respect for the person's principle of autonomy. * Participate in monitoring the evolution of the client's situation, in collaboration with the client, the entourage, the interdisciplinary team and the treating physicians. * Provide clients with services such as basic care, nursing care, household help and family help. Profile: * Hold a CFC as ASSC. * Have, if possible, experience in home care. * Be available to work during the day, evening and weekends. * Be able to work autonomously and in an interdisciplinary manner. * Hold a driver's license and possess a vehicle (possibility of using a professional vehicle).
