We are looking for the Centre médico-social (CMS) de Moudon for an: ASSC ON HOURLY CONTRACT (evenings and weekends) Mission: * Collaborate on the medico-social care project (support in the form of assistance and care) based on respect for the principle of the person's autonomy. * Participate in monitoring the evolution of the client's situation, in collaboration with the client, the entourage, the interdisciplinary team, and the treating physicians. * Provide clients with services such as basic care, nursing care, household help, and family help. Profile: * Hold a CFC as ASSC. * Have, if possible, experience in home care. * Be available to work mainly in the evening and on weekends. * Be able to work independently and interdisciplinarily. * Hold a driver's license and possess a vehicle (possibility of using a professional vehicle). * Knowledge of German would be an asset.
