Head of Social Services and Case Management

Contribute

• Personnel, professional, and economic management as well as the further development of Social Services and Case Management
• Ensuring and further developing the psychosocial counseling and support of patients as well as their relatives in complex social and personal situations
• Professional guidance and support for social insurance law issues as well as ensuring need-based and sustainable follow-up solutions.
• Professional support and advice to the team in complex issues and challenging case situations
• Strategic and organizational further development of the area including processes and key figures
• Development and establishment of an interprofessional, early-intervention discharge management, from hospital admission to secured follow-up care.
• Active shaping and further development of the interfaces to clinics, nursing, medical staff, and capacity management
• Maintenance and expansion of the network with general practitioners, Spitex, nursing institutions, rehabilitation clinics, authorities, and payers
• Development and establishment of cooperations and cross-sectoral care pathways
• Driving digitalization and the introduction of suitable digital solutions for referral, case coordination, and transition

Bring

• Degree in Social Work (FH/HF), nursing, or a comparable training, further training in Case Management and leadership
• Several years of experience in the management of a social service or case management, ideally in an acute care hospital
• Sound experience in psychosocial counseling and support of patients as well as in complex social work and social insurance law issues
• Sound knowledge of the social insurance system and the regional care landscape
• Experience in organizational development and change projects
• Strong network and interface thinking as well as negotiation skills
